Qualcare Employer Reporting forms are being sent to groups enrolled in The Affiliated Physicians & Employers Health Plan
Employers enrolled in The Affiliated Physicians and Employers Health Plan (APEHP), a self-insured MEWA Health Plan, will be required to file the IRS Employer Reporting Section 6055/6056. This reporting intends to determine that an individual has acquired or has been offered Minimum Essential Coverage from their employer as required by the Affordable Care Act.
