2017 New Jersey Employer Certification Form Plan sponsors required to complete new certification form prior to renewal. Effective January 1, 2017, New Jersey changed the definitions of “employee” and “small employer” to align with Federal law. As a result, the annual New Jersey Employer Certification form has been revised. In order to obtain a 2017 Small Employer Health Benefits Plan, plan sponsors must certify their status as a small employer on this new form. A copy of the new form is being sent as part of the January, February and March renewal packages.  A separate mailing will be made for later renewal dates.

IMPORTANT: Even if plan sponsors already submitted a prior version of the form to Aetna, they must complete and submit the new form. If the plan sponsor does not return the New Jersey Employer Certification form at least 10 days prior to their renewal, we will non-renew their Small Employer Health Benefits Plan.
