While the Ultimate Health policy requires that members file a claim within 90 days after the covered expense has been incurred, it also recognizes this isn’t always possible. As a courtesy to members, claims are accepted up until 90 days after the Ultimate Health plan year ends.
 
Example:
Date of service for claim: 7/1/16
Ultimate Health plan year: 1/1/2016 to 12/31/2016                                                                                        
Last day to file claims: 3/31/2017
 
Please encourage members to gather claims incurred in the 2016 plan year and file them now. If their Ultimate Health plan year started on 1/1, their claims will not be accepted unless they are filed by 3/31/2017.
